Comparing Anal Fissures and Hemorrhoids

Overview

Hemorrhoids are swollen blood vessels in the anus or rectum. They can cause itching, pain, and bleeding. On the other hand, anal fissures are small tears in the thin skin that lines the anus. They can be painful and may also bleed during or after a bowel movement. Hemorrhoids are more common than anal fissures.

Here are some of their similarities and differences:

Symptoms:

Both hemorrhoids and anal fissures can cause rectal bleeding and discomfort. Pain with hemorrhoids is usually mild whereas anal fissures are likely to cause more pain. Other symptoms include:

  • Hemorrhoids: Anal itching, noticeable lump in the anus
  • Anal fissures: Anal itching or burning that doesn’t go away

Causes and Risk Factors:

Hemorrhoids usually are caused by too much pressure on the anus. Factors that increase the risk of developing hemorrhoids include:

  • Straining during a bowel movement.
  • Sitting for a long period of time, especially on the toilet.
  • Chronic constipation or diarrhea.
  • Obesity.
  • Engaging in consistent heavy lifting or activities that strain the body.
  • Pregnancy (enlarged uterus presses on the vein in the colon).
  • Anal sexual intercourse, which can irritate hemorrhoids.
  • Anal fissures are caused when the skin of the anus gets torn or cut. Factors that increase the risk of developing an anal fissure include:
  • Inflammatory bowel disease
  • Poor blood flow to the anus and rectum
  • Anal intercourse
  • Problems of the anal sphincter muscle
  • Anal cancer

Treatment

In many cases, hemorrhoids and anal fissures resolve on their own by reducing pressure on the anus and self-care with sitz baths (soaking the anus in a tub of warm water) the over-the-counter topical ointments or creams to reduce swelling and itching.

Hemorrhoids that do not go away may require surgical removal. Anal fissures that do not heal may require surgical repair or removal.

It's important to note that while bleeding from the rectum is more likely to be from hemorrhoids, an evaluation by a doctor is important to determine the exact cause of the symptoms and appropriate treatment recommendations.
